    Medical Report Application

     Patients / Next of Kin / Insurance Agents may request for medical report application at Health Information Management System (HIMS) Services via walk-in / e-mail / post.


    HIMS personnel will process all the necessary documents before we hand them over to the respective Consultant / Medical Officer.  ‘Consent for Release of Medical Information (Medical Report)’ must have a valid written signature from the patient before we release it to any third party.  Please be informed, that we only accept the original consent and will thoroughly check the validity of the signature of the consent form before releasing the medical report. The consent form may be obtained at the HIMS Counter at Level 1, KPJ BMSH or requested via e-mail.


    The process for the report to be completed is within 4 weeks.  There are cases where requests may take longer than the stipulated completion period. Causes of delays in prompt processing include:

    • Unavailability of doctors (e.g. away / on leave / no longer practising)
    • Patients having upcoming clinic appointments (follow-up)
    • Patients are still admitted / hospitalized
    • Patients have multiple reports requested from several doctors
    • Doctor’s limited no. of sessions at KPJBMSH (e.g. only one session per week)
    • Doctors are visiting doctors without clinic sessions at KPJBMSH
    • Specialist Medical Reports cases may require longer processing time (approximately 30 working days) as a review at the Specialist Outpatient Clinic may be required after the patient has been discharged or an open date given from clinic review.


    Kindly take note that, a full fee will be collected upon completion of the report. You will be notified via phone calls once the report is ready to be collected. 


    Meanwhile, a full deposit will be collected from the law firm who requested any written Medical Report / Initial Report / Specialist Medical Report.

    b)  CONSENT FOR RELEASE OF MEDICAL INFORMATION (MEDICAL REPORT)

    Mandatory Requirement by PHFS Act 1998 [and its Regulations 2006] and PDPA Act 2010 [and its Regulations 2013]: -

    1.  A patient’s written consent form is required for the process of disclosure of personal and sensitive clinical data.

    2.  Valid written consent under sub-regulation (1) shall be obtained from:

    a)  The patient (in person)

    b)  If the patient is mentally or physically disabled, the spouse, parents or next of kin; or

    c)  If the patient is unmarried and below eighteen (18) years of age, the parent or guardian

    d)  If the deceased patient, consent from the appointed representative or executor of the estate is required.

    • A copy of the death certificate and other supporting documents, e.g. marriage certificate and letter of administration, are required as proof of the representative's relationship to the patient.

    3.  A copy of the patient's NRIC / Passport Identification document.

    4.  A copy of the supporting document (NRIC / Passport) from a third party (parents/spouse/next of kin/insurance agent/lawyer firm).



    Medical Information

    Requests for copies of investigation reports / medical information will be processed in 3 - 5 working days from receipt of request.

    • Discharge summary

    Kindly take note that, all of the requests will only be released with a duly signed of the ‘Consent of Release of Medical Information’ by the patient.
