As we monitor carefully the global impact of COVID-19, Encoremed has developed a digital online declaration form for patients to fill in before coming to the hospital (via online).
All patients must fill up Covid19 Declaration form (via link provided) before making online appointment and then will receive a QR code once completed with regular updates from the hospital via SMS.
