    Registration

    We really encourage patient to set appointment prior visit to the respective services. The registration counter for each respective service is located as follows:

    i) KPJ KL Dental Specialist Centre – Level 3
    ii) Haemodialysis Service – Level 3
    iii) KPJ KL Rehabilitation Centre – Level 2
    iv) Consultant Clinics - Level 2

    During registration for admission, you will be requested to present your identity card/passport or any other legal documents for registration purpose. For children under 12 years old, parents or guardian's identity is required for registration. Upon completion of registration, you will be requested to check and verify your particulars on the printed Admission Registration form. For return patient, you are advised to update your particulars and contact information during registration.
